Ordering HLQ and PPF Forms

Your school will receive a red HLQ/PPF Daily Mailer.   It is important to open and check the contents of the mailer and return it to the Test Processing Center on a daily basis with all newly completed HLQ/PPF forms.  HLQ/PPF forms should no longer be sent to the ELA Department.  They must be sent directly to the Test Processing Center in your red HLQ/PPF Daily Mailer.

The HLQ/PPF forms have not changed.  However, there is a new process for ordering the forms.  HLQ/PPF forms will be ordered through the Test Processing Center and not the DPS Warehouse.  Please use the attached  Additional Material Request Form [pdf] and fax it to 720-423-4269. Your order will be sent the following day in the HLQ/PPF Daily Mailer.  The forms are now in scannable form.  Each form costs 3 cents. At the end of each semester, schools will be sent an itemized invoice and will be billed at that time.
